Faculty & Student Injuries - What To Do

The information below is for injuries sustained while working for the university, completing clinical rotations, students teaching, or social work internships:

1. Read the following links:

2. Download and complete the following forms:
   • Supervisor's Report Form of Student Clinicals and Work-Related Accident/Illness (.pdf)
   • Employee's Claim Form for Workers' Compensation Benefits (.pdf)
   • Sharps Injury Log (.pdf)
   • Authorization and Return-to-Work Letter for Sharp Rees-Stealy Occupational Health Center or Pharmacy (.pdf)

     (Forms will indicate "Employee", but students need fill out that portion of the Workman's Compensation form.)

3. Bring completed forms to the School of Nursing office in Hardy Tower 58.
